HOW TO REQUEST A TRANSCRIPT OR DIPLOMA

STUDENT TRANSCRIPT & DIPLOMA POLICY
1. Student Transcripts/Diplomas are free of charge.
2. Official Transcripts will bear the Department of Education seal and signed, in ink, by the Manager of High School Certification or designate.
3. A student/former student’s Transcript/Diploma is only released if:
- A Transcript/Diploma request form is fully completed.

- Transcript/Diploma requests must come from the student/former student. Requests from a third party will be processed only with written permission from the student/former student. The Consent to Disclose Transcript form must be completed. Consent to Disclose Personal Information Form PDF (613 KB)
- The request is made using regular mail or email (request made via telephone are not accepted).
- A person who is 19 or older must be the individual who completes the request form and signs it.
- A parent can only request their child’s transcript if their child has not yet reached the age of 19.
- Transcript/Diploma requests will be accepted from Universities/Colleges if the request has a signed student authorization form attached.
4. A transcript can be either mailed or emailed from the Department of Education to a recipient as indicated on the transcript request form
SERVICE STANDARD
Upon receipt of a completed application for an official high school transcript, the department will respond within five business days. Due to the high volume of requests in August/September and January/February the department will respond to the applicant within ten business days.
